Leave of absence

1. To request a leave of absence one must use  the Janus System, sub-item "Requests".

2. The system itself will direct you to a new page containing the student's information and the type of request you want. If you choose to cancel your registration, this should be the option you select.

3. 3. The request for  a leave of absence must be accompanied by a description/justification stating the reason for the request (health, pregnancy, professional, family health), as well as the following attached documents:

a) Letter from the student, addressed to the Program Coordinating Committee (CCP), containing the reasons for the request documentally proven, intended deadline and start date of the leave of absence;
b) Detailed opinion from the advisor, addressed to the Program Coordinating Committee (CCP), stating the request and confirming the intended deadline and the start date of the leave of absence;
c) Proof: 
- Birth Certificate (Maternity Leave); or
- Employer Statement (Professional Reason); or
- Medical Certificate (Health Reason) containing the ICD (disease identification code), the need for the leave and the indicated period (maximum of 12 months) and the indication of the starting date (from ...);

d) Schedule containing the activities that will be developed after the leave of absence period

IMPORTANT:

- The maximum period of leave of absence is 365 days (12 months), according to USP's Postgraduate Regulations.
- It is possible to cancel the registration during the period of validity of the extension of the deadline for the conclusion of the dissertation or thesis, exclusively in case of illness. 
- The leave of absence may be backdated to the date of the occurrence of the reason for its granting, provided it is requested and while the reason persists, and provided it does not overlap with any activity carried out, except registration.

Some comments about the documents required for the request of leave of absence:
1. 1. In cases of health-related cancellation requests, the Medical Certificate issued by the accompanying physician must contain
a) The CID of the illness
b) The need for the student's leave, as well as the indicated period for this leave (12 months at the most)
c) The starting date of this leave: "from...".

2. In cases of professional reasons, a certificate from the employer must be attached informing the period in which the applicant will be away from his/her graduate activities and explaining the reasons or professional project in which the applicant is involved and that will affect the continuity of his/her graduate studies.
